Patient Comment: While I' m sure she is more than competent as a
physician, I can tell you from personal experience that her office practices are
deplorable. I had a 10:00 am appointment with her last August, for which I arrived at 9:20
to fill out paperwork. At 11;45 I was taken to the back for weght, blood pressure, etc. At
12:30, the office staff adjourned for the lunch they had catered for them courtesy of a
drug rep Smelled good, too. At 2;00, when I inquired as to the status of my 10:00
appointment, I was informed that Dr. Horsley likes to give her attention to each patient,
and won't I appreciate that when it's my turn?! At 3:15 I informed the receptionist
I was leaving. I have NEVER been treated so discourteously in a physicians office--why
bother with an appointment system at all if it's never going to be honored? I have since
found a caring, thoughtful doctor, but I would caution you regarding a recommendation for

Life can be a pain. Aches, pains and the minor discomforts of life can't always be avoided.
But what about pain that disrupts your daily routine? The kind of pain that keeps you from living a fully functioning life; from being able to work; interrupts your sleep; and can't be easily remedied?
That's called chronic pain, and it can have a devastating effect on your finances, your job, your social life, your family life and your mental health.
Where do you go when the doctor can't find the source of what ails you and you've tried every remedy known to man? For many, the next step is "pain management."
But what is pain management, and why is it necessary?
"It's necessary because, unfortunately, there's not much training in medical school to address and assess pain," said Dr. Michael C. Scannell of the Medical Pain Clinic of Monterey. "It's now considered the fifth vital sign. It's been a black hole that we didn't have a grasp around; something that physicians pushed aside until now. Pain management is a combination of comprehensive education on the origin of pain, differential diagnosis of chronic pain conditions and the integration of the mind, body and spirit."
Scannell is a board-certified anesthesiologist who took time off from private practice to enter a year-long fellowship program at Stanford University's Pain Management Program, one of the few pain management fellowship programs in the country.
Scannell explains that pain tells a person there is ongoing tissue damage or harm to the body. Often chronic pain is unique in that it's really an abnormal response; the body is continuing to show that there is harm despite no ongoing tissue damage.
Part of the education process to the patient is to let them know that there is not ongoing harm to the body and that chronic pain can exist due to a malfunctioning of the nervous system, Scannell said.
"It's important for patients to understand that sometimes there is nothing catastrophic going on," he said. "Patients come in looking for the next
step. They doctor shop and end up with unnecessary injections, surgery or medications, which can cause more harm to them. After any ongoing tissue trauma is excluded, the focus is shifted from looking for a cure for chronic pain to managing with chronic pain. Pain isn't strictly tissue trauma."
Pain caused by small things can be catastrophic and take on a life of its own in the patient. Small sources of pain can lead to huge loss of function. Many layers of problems feed upon each other such as financial stress, medical bills, depression and a loss of self. Each patient is unique and there are no set timelines when it comes to managing their pain.
In addition to living with pain, patients also have the underlying stigma of drug addiction and doctors are often leery of treating pain patients. However, Scannell said it comes down to the difference between a physical dependence and a behavior process; one allows a patient to live with pain, the other is the driving force of the patient and the addictive patient shows up fairly quickly.
"Physiologically, everybody can become dependent and withdraw off narcotics, but addiction is behavioral, it's a craving. Addiction is continued use (of medication) despite harm," he said.
There are some things to look for when seeking help. For instance, Scannell advises seeking out a multi-disciplinary pain clinic that does everything, not just injections.
He also said to look for physician training, someone who is closely integrated with fellow community physicians, physical therapists and alternative medicine such as acupressure or acupuncture and someone who has a referral base that includes these aspects as part of the care plan for the patient.
A good pain management physician must have an infrastructure of a narcotic contract and close, personal contact with the patient and the patient's family to educate them on terms of appropriate narcotic use.
"My draw to pain management is the challenge," Scannell said. "I value communication, and I try to form treatment plans with patients; I get a lot of enjoyment from taking patients who have had problems in the past and seeing their success and improvement. You have to enjoy success over time. It's a long-term process and requires commitment on everyone's part."
Scannell marks a successful patient as someone who can show improvement in daily function, in their mood, compliance with treatment plans and ultimately, "someone who can live according to their values and goals with a chronic pain problem."
